ID cards will be issued to all non-probationary EMS agency personnel. In order for an ID card to be made, the ID card request form must be filled out completely and signed by the requester’s Company Chief. The Chief’s signature is required in order to verify that the information given by the requester is correct.
No ID cards will be made for probationary members, for fire related certifications, or other administrative positions such as President, Secretary, etc.

Information provided on the ID card:

  • Company Name
  • Requester’s First & Last Name, middle initial is optional
  • EMS certifications, driver status
  • Requester’s Photo
  • Requester’s County Patch
  • Office held (Chief only)

Replacement Fee
The Council no longer charges a fee for replacement cards. 

The following certifications will be provided for the card:

  • EMR
  • EMT
  • AEMT
  • EMT-I
  • EMT-P
  • Driver

Individual Cards
If you need an ID card made, stop by the office anytime Monday thru Friday 8:00am-4:00pm.

If your company needs IDF cards made for less than 15 people, please email Janet at  jspilewski(@)vaems.org to schedule an appointment. If there's a group larger than 10, arrangements can be made to bring the equipment out to the individual stations.
